As part of the in-home care, we cater to skilled and non-skilled services generally ordered by physicians for clients after an illness or injury that requires care but not hospitalization. Patients have the liberty of choosing their agency.

Our objective is to help patients get better, regain their independence, and become as self-sufficient as possible.

Eligibility Criteria

  • Contact your local social services department to see if you or your loved one is eligible for the CCC+ program and obtain a UAI screening. If you already have a UAI, contact us today to set up an appointment with one of our nurses to conduct a screening and better determine your or your loved one needs.
  • Contact your primary care physician and talk to him/her about setting up home health services. Your physician will determine if you qualify and meet the homebound status criteria and what services are needed.
  • As a patient, you have the freedom to choose your agency, or you can ask your physician for his/her recommendation. Contact the agency to determine insurance coverage and limitation.
